Licensed Practical Nurse (LPN)

LPNs at CONSTANT HEALTH CARE AND WELLNESS SERVICES have been trained to actively take part in the recovery and/or health management of a home bound patient. They can provide home nursing services under the direction of your physician:
- General Monitoring of Health Conditions
- Assisting the Client with Feeding / Eating
- Assistance with Toileting
- Assistance with Activities of Daily Living
- Transportation to Family Gatherings
- Light Household Tasks
- Assistance in Maintaining a Home Exercise Program
- Overseeing Safety for the Client at Home
- Simple Wound Care and Dressing Changes
